From 4 to 5 on the 12A_7 heaters or pin 4 to ground.
One comes from the CT the other is grounded.

You asked for a schematic the other night. Here's just the power supply. I hope that makes things clearer. [link] Sorry for the mixed drawing, but I couldn't find schemtic diagrams for V-regulators.

The power supply is slightly different without regulated plate supply and w/ series heaters. On the second page the schematic looks like Stephane's Sopht amp or an early version thereof.
